Health & Safety Issue Observed	damaged surface (ie: wall, ceiling, flashing, drywall, flooring, stairs)
Health & Safety Issue Details	The hallway east of the boiler room is falling apart. The concrete is crumbling, temporary support beams have been in place for years and are rusting. This area supports the stairs going into the main entrance and is disintegrating.

Corrective Action	Put in an urgent notification for the ironworkers to repair the support beam. Notification number: 13148308. Also informed the FTL, CTL and MTL about the issue - (acting HC).
