Health & Safety Issue Observed	extension cord/power bar - inappropriate use of extension cord/power bar (be specific)
Health & Safety Issue Details	Several rooms have extension cords used as permanent wiring. Electrical Code does not permit the use of extension cords for permanent wiring. Of course the ideal fix would be to have electrical drops and receptacles every 6 feet or so----but that ain't going to happen. So, the workaround is to unplug the extension cords at the end of the day. There is an extension cord powering the LCD projector in this room.

Corrective Action	Principal and Caretaker to purchase necessary materials or reconfigure wiring. If repair work is necessary, generate SAP Notification (record the number).
Corrective Action Taken	The P/VP followed up to make sure these rooms have proper wire covers/guards, and will remind all staff that when the cords are not in use to unplug them.
