Health & Safety Issue Details	Teacher noted the inconsistencies of phone working sometimes caller cannot hear her, OA noted will follow up with teacher.

Corrective Action	Principal to review with Family Team Leader to ensure a communication plan is in place. PA's and telephones are not required in 'service rooms'.
Corrective Action Taken	Teacher was not hanging up the phone properly. Reminded teacher to make sure it was on the hook properly.
