Health & Safety Issue Observed	damaged surface (ie: wall, ceiling, flashing, drywall, flooring, stairs)
Health & Safety Issue Details	A few spots on the floor in this area where the tile has chipped leaving large enough gaps to cause a tripping hazard. Advised the tile be replaced. In addition to this - the split strips on the stairs are peeling off on some steps

Corrective Action Taken	a notification has been put in for the carpenter to replace the tiles in question, they have looked at the tiles but has not yet fix the problem
