Health & Safety Issue Observed	leaking/pooling/ponding of liquid (specify liquid and source if possible)
Health & Safety Issue Details	There is leaking of water via a chained metal rusted roof (lid) in a section of the boiler room; that roof (lid) can easily be lifted by students on the outside (playground) when doing so, this also presents the hazards of getting hands/feet trapped as when the lid is lifted, there are gaps . This was sited a year ago on a previous inspection (Nov 17, 2020) and work order placed.

Corrective Action Taken	i put in notification sept25/20 #13533870 for the grate to be welded shut nobody has come by for this to my knowledge
