Health & Safety Issue Observed	safety equipment and systems are not accessible, blocked or damaged (ie; fire pull station, fire hose, panel, gas feed, water shut off, blocked breakers)
Health & Safety Issue Details	Carbon Monoxide CO Detector needed for Boiler room. For reference TDSB Guidelines GU.FAC.044 Spills and Leaks on carbon monoxide. https://policies.tdsb.on.ca/uploads/files/live/35/1858 Boiler is located besides caretakers office and besides academic space. Air handling units (fresh air intake plus air circulation) are switched off at 6pm and turn back on 7am (Building Automation Systems) that the Head Caretaker has no control. It is detrimental on having it installed. Once installed it should be dated and calibrated yearly.

Corrective Action	Principal or Caretaker to investigate the concerns. If further action is required, contact your Facility Team Leader or Regional OH&S Officer.
Corrective Action Taken	Carbon Monoxide Detector is not working and i was told in the past H&S reports that the Board will not repair or replace italso EF's are turned off at 9pm and AHU at 8pm
