Health & Safety Issue Details	Extension cord used as permanent wiring. Notice for Principal: Please remind staff that extension cords cannot be used as permanent wiring, and if there's no other choice but to use them throughout the day, they will need to be unplugged at the end of each working day. Also the cord is a potential trip hazard. Two solutions: unplug cord at the end of 7each day; and use a wire trough throughout the day. See photo.

Corrective Action	Principal and Caretaker to purchase necessary materials or reconfigure wiring. If repair work is necessary, generate SAP Notification (record the number).
Corrective Action Taken	Principal has notified teacher in the classroom to unplug the extension cord at the end of each day as it is not to be used as permanent wiring.
