Health & Safety Issue Observed	Guide to Occupational Health & Safety Reporting Forms
Health & Safety Issue Details	Please note that this document changed names a few months back and has been updated. Please post the new version which is found on tdsb.on.ca by clicking on Occupational Health and Safety Reporting forms and then found at the top of that page. It's not found where you'd think it would be with the other documents required for the bulletin board. The new name of the document is Guide to Occupational Health and Safety Reporting Forms. As well, the JHSC Minutes need to be updated each month. Those currently posted are not the most current. It is also recommended to include the year of certification for staff trained in First Aid. Please see attached photo of checklist which was also pinned to the board for your reference.
