Health & Safety Issue Details	anchors and screws holding the shelf are pulling away from the wall.

Corrective Action	Principal to tag out equipment until repair is complete or remove equipment from the workplace.
Corrective Action Taken	I have replace these screws and anchors and the teachers still over load these shelves with heavy math books at the end of the year. They were supposed to order new shelves to be installed two years when I talked to the Head of the Math Dept.
