Health & Safety Issue Observed	extension cord/power bar - inappropriate use of extension cord/power bar (be specific)
Health & Safety Issue Details	Wires on the floor, even those temporarily so, need to be protected with PROPER covers to prevent slip, trip, and possible shock, hazards. Taping the wires to the underside of the cover will help to prevent the cover moving off of the wires if the cover is moved.

Corrective Action	Principal and Caretaker to purchase necessary materials or reconfigure wiring. If repair work is necessary, generate SAP Notification (record the number).
Corrective Action Taken	The proper cover is in the room. Will speak with the classroom teacher to tape the wires to the underside of the cover.
